Payment Procedures

On the day of your visit, you will be responsible for any co-payment, material, or procedure that is not covered by your insurance. It is a patient's responsibility to know what routine vision benefits and materials are covered by their insurance and/or vision plan.

Contact Lenses & Optical Goods

Payment is expected in full day of for any materials ordered or dispensed unless other arrangements have been made, or our billing staff has verified your insurance coverage.

Surgery & Insurance

Our insurance department will do their best to assist in determining insurance coverage and estimate balances once insurance is filed on major surgeries. If certain financial circumstances exist, our billing department will be happy to help you with extended payment plans or Care Credit applications.

Prior Authorizations

Many HMOs (health maintenance organizations) require patients to obtain an authorization from your primary care physician or they will not cover services rendered by a specialist. It is the patient's responsibility to provide Ozark Eye Center with this prior authorization if one is required, otherwise the HMO will deny our claim and the service's payment will be the patient's responsibility.

Payments Accepted

We accept Visa, MasterCard, Discover, and Care Credit, along with cash and checks. For large balances on surgeries or optical goods ask us about Care Credit payment plans that are available here.
